This article reviews the medicolegal requirements for documenting forceps and vacuum extraction deliveries, highlighting the inadequacies in current hospital forms. The authors conducted a study involving 21 hospitals in southwestern Ontario, revealing that less than 50% of the necessary documentation requirements were met. A new generic form was developed to improve compliance and assist perinatal committees in enhancing documentation practices. This work is particularly relevant for rural medicine, where standardized documentation can aid in reducing legal risks associated with assisted deliveries.
